What is Blepharoplasty?

Blepharoplasty, or cosmetic eyelid surgery, remains one of the most requested aesthetic facial procedures. The surgery was first popularized in the mid 1800's, when it was performed for the removal of excessive upper eyelid skin. In the early 1900's, the surgery for the lower eyelid was developed and quickly became accepted. Blepharoplasty is used to enhance the appearance of the eyes by removing excess skin and fat from the eyelids. Eyelid surgery can be done in conjunction with other procedures such as facelift and brow lift to achieve the desired look. Dr. Kassir will advise you on what you will benefit from the most.

How is Blepharoplasty performed?

Dr. Kassir performs Blepharoplasty eyelid surgery in his surgical clinic in Wayne, NJ and 799 Park Avenue, NYC offices. He usually performs with local anesthesia and IV sedation. The procedure can take less than 45 minutes. Incisions are made along the eyelids in inconspicuous places: in the natural creases of the upper lids for upper eyelid blepharoplasty and just below the lash line on the lower lids for lower eyelid blepharoplasty. If no skin needs to be removed, Dr. Kassir will likely perform a transconjunctival blepharoplasty, where the incision is made inside the lower eyelid and there are no visible scars. In all cases, the surgeon then removes excess skin from the eyelids using a scalpel, laser or radiofrequency tool. Fat may be removed as well.

Recovery from the Blepharoplasty

Bruising, swelling and a sense of tightness in the eyelids are common after eyelid surgery. These should subside within 2-4 weeks. Most patients resume activities in 3-5 days, when non-absorbable stitches are removed, and return to work in about 10 days.

By one to two months after surgery, you should see the true results of your eyelid lift: a more awake and cheerful look around the eyes, and greater self-confidence. Contact lenses may not be worn for two weeks. Eyes can be sensitive to light and wind and may be slightly irritated for a short while; your surgeon may prescribe eye drops and cool compresses to ease any discomfort.

Upper Eyelid Surgery

The purpose of the upper eyelid surgery is to remove the redundant skin that overhangs the crease. The procedure is performed through an incision that is placed at the eyelid crease, so that it is completely hidden. The skin is marked carefully before surgery to ensure that the exact amount of skin is removed. The skin is removed and then a small amount of fat is removed. The incision is then closed with a tiny continuous stitch that is placed inside the skin to ensure that the incision heals well and is virtually invisible. The stitch is then removed on post-operative day five.

Lower eyelid surgery

The goal of the lower eyelid surgery is to provide a smooth lower eyelid contour that blends with the cheek. This is done by removing the fat that protrudes underneath the eye, removing some of the excess skin, and then elevating and repositioning the eyelid muscle. This must be done in a manner that will maintain the patient' s natural eyelid shape and avoid any lower eyelid position that can occur from a poorly performed blepharoplasty.

Risk of Lower and Upper Eyelid Surgery

There are few risks involved from lower and upper eye lid surgery. They can include infection, reaction to anesthesia, dry eyes, double or blurred vision for a few days, temporary swelling of the eyelids, tiny whiteheads, and difficulty closing eyes when going to sleep. Uneven healing and scarring, and pulling down of the lower lids are very rare and may require surgical correction. NJ and NYC Patients with thyroid problems, dry eye, high blood pressure, diabetes, detached retina, glaucoma or other health problems should consult with an ophthalmologist and Dr. Kassir about eligibility.

Recovery from Lower and Upper Eyelid Surgery

The patient is discharged home the same day after lower and upper eyelid surgery. The first night should be spent relaxed. Activity should be kept to a minimum. A liquid diet is best for the first eighteen hours after anesthesia. Pain should be minimal. Tylenol is usually enough to control any minor discomfort. Additional pain medication will be provided, in the event that it is needed. Any significant pain should be reported to Dr. Kassir immediately, as this may signal bleeding under the skin. Some mild bruising may be visible around the eyes. Most patients return to work within three to five weeks after lower and upper eyelid surgery. Most of the swelling resolves in the first three weeks. It is recommended that physical activity, such as running or lifting to be resumed three weeks after the surgery.
