Our practice is not in-network with any insurance provider and will require out-of-network benefits. If you would like to find out if your insurance plan covers out-of-network expenses, please reach out to your respective provider. Some procedures that are deemed medically necessary may be covered by certain PPO plans with an out-of-network benefits option for specialists/surgeons. In those cases, anesthesia fees and fees related to the outpatient surgical center may or may not also be covered by the PPO insurance plan.
Consultation fees will be applied to your procedure should you decide to go ahead with surgery on the day of your consultation. Consultation fee is non-refundable.
